After your assessment, we’ll get you moving in the right direction fast with 3 additional meetings in the next 6 weeks.

    People are most successful with making change when we focus on one priority at a time. Each time we meet, we’ll work together to problem-solve and plan your next step.

    • A 45-minute meeting to plan the one change you’ll focus on for the next two weeks. We’ll also agree on the focus for our next meeting.

    • 2 weeks and one 3-day food diary later…

    • A 90-minute Focused One Goal Session to tackle the biggest problem that’s holding you back (meal planning, smart shopping, eating on the go, or a stocking a healthy kitchen). If your biggest challenge isn’t one of the four Focused Fix topics we’ll focus on what is.
    • 2 weeks and one 3-day food diary later…

    • A 45-minute meeting to celebrate what’s working for you and troubleshoot what isn’t. We’ll plan your next steps, whether it’s continuing your work with me, connecting you with other services in the community, or moving forward on your own.

    Note: You’ll need a Heart Health Nutrition Assessment first, if you haven’t had one in the past year.
